What recourse do I have if my insurance company does not pay claim?
My insurance company received a claim from a doctor for services rendered in 2005. Am I liable if my insurance company doesn't pay?

Yes, you are liable. And an insurance company will not pay for services rendered in 2005 and just now filed. Most insurance companies have a 12 month cut off, some less. That means the claim must be filed within 12 months of the date of service for the insurance company to consider payment. You will end up paying the claim.
