How do you contact the head insurance headquarters?
I had a paid up policy some years ago for 2,000 and how do one contact the head insurance office to find out if there is a policy on file? Also will this help as I also forgot the name if the insurance company.

The "head insurance office" is not going to be found if you don't know the name of them!!
If you're asking for one governing legal authority, there isn't any. Each state is pretty much autonomous.
Your best bet is to go through your old bank records and see who you paid, "some years ago", then go from there.
